Ive only found him to be stupid with healing you. Other than that its your lack of skills with using the d-pad controls or knowing when to use them for certain situations.

It's part of the downloadable content available from EA. It's free on the Xboxlive marketplace. You get Kiev and an alternate ending Miami mission.

No one has said for sure as far as I know as to why Rios and Salem don't say things when they pick up the cases.
They might have just thrown them in at the last minute. Tis a shame though, would have made a decent additional achievement.
To htah427, you could always grab the cases on the easier settings or kill every last mo fo on the level before you look for the case. You get the same amount of cash no matter what the difficulty is set to as far as I know.
