Wasn't this also released by American Video Games? I have an old flyer from them with this on it as well as some Terminator looking guy on the front. They sent it to me with the whole "Ban REV 11" Nintendo lock-out chip info pack which included an alligator clip and sticker. :P It was handy because it's the same time when Nintendo released a NES that started blocking unlicensed games and it was easy to bypass with a bit of solder.
