Gosh, this song never made me cry before, but reading your English translation made me actually listen to the lyrics, which I'm not sure I ever did before.
IMO, this song is about unconditional love, that you (the lyrical ego) would do anything (and even more than that) for someone you love more than anyone/anything else in the world.
You are willing to sacrifice yourself if that's what it takes to rescue the person you love. To me, it sounds like the person the lyrical ego is talking to is in big trouble, maybe they've messed up their life and they need someone (the lyrical ego!) to help them back on their feet.
The lyrics are very touching indeed.

"über den Fluss" --> to the 'other' side, maybe referring to life after death?
"So oft gehen die, die noch nicht weggehen wollten" -> those who die 'before their time'
Hoer ich das falsch oder singt sie "Am Ende des Wegs" - dann waer das eigentlich "AT the end of the road, if I have to, I'll carry you 'cross the river" - oder?

Ich glaub nicht, dass das gegen dein Deutsch spricht.
So richtig ueberdeutlich versteht man das ja nicht - ausserdem hat man beim Hoeren vielleicht eine gewisse Erwartungshaltung.
