  • As someone specializing in orthopedic surgery, I can tell you that this practice violates many guidelines imposed by such organizations such as the ADA. Such restraint is almost never used EVEN on patients who suffer of Peristent Developmental Disabilities, and here we have normal children being restrained. It's tragic that patients are being restrained for the dentist's convenience, the dentist's convenience being valued more than the patients comfort.

  • when I was five in 74', I split my chin open and i was scared as it is...and I was tied down on a papoose board....I still remember that day and it has caused me "psychological" distress....i remember being tied down and before they put stitches on my chin, i remember a blue cloth covering my face...but I still was traumatized by that experience!!!!

  • Poor little miguel... I took my son to the dentist yesterday for the first time. I allowed them to put him in this and once I saw him in it changed my mind. It seemed to horrific. How can we all do something to make them stop this? That poor child (Miguel) and all those others who we dont know of.

  • im Canadian and this is insane you can not go anywhere in Canada and get dental work without anesthesia or freezing. It's considered torture otherwise. And I have been to the dentist and orthodontist many times throughout my life and childhood and i have NEVER seen one of those torture boards!

  • I wouldn't be surpized if half those kids develop PTSD. My parents were always allowed in the room with me at the dentist. The dentist would even look on me if a parent was not in the room with me. I never had a problem with the dentist and I am autistic. If a doctor or someone dosen't let a parent in the room with their child, that should set off bells and whistles. Animals aren't even restrained for dental treatments...they are sedated.

  • @hyrizredneck Like I just posted in my comment, I was in my teens and they needed to do this to me for my safety and theirs. Not being able to move really added to my anxiety and made the anticipation all the worse. It was also humiliating. I felt like a prisoner but at the same time only the staff saw me tied up & I understood it was for my own good and I consented to it in advance at my consultation. They also showed me exactly what they were going to do and let me try them on in advance.

  • @rainbowglue2 I can't for medical reasons. Believe me, I would have much preferred that. The thing that a lot of ppl don't realize is it is not safe for everyone to do. It was humilating and I didn't want even my mom to see me all tied up like that. I had to go in about 10 minutes early so they could do the straps and all that and then had to lay there not being able to move and lie there and think about what was going to happen. They of course did numb my mouth first. At least I had my ipod

  • It can be traumatic, especially very young children but they had to use one of these on me and I was in my teens. I have a really over the top fear of needles and the dentist and I get so scared I kick and flail and I can't control myself. There are also medical reasons that I can't be fully sedated. We met with my oral surgeon and discussed this and he suggested we do this. It's not common at all to use these on adults or teens but in my case it was necessary for my safety and the staff's.

  • @busybee, im speaking from experience, they treated me and my brother the same way after yelling at us to stop crying, they would forcibly put me on the chair and strap me down. and then, the dr. introduced himself then not even numbing my mouth they drilled my teeth and i was screaming and crying small smiles is the worst place ever!!!

  • @ hydre, umm the kid is obviously uncomfortable and in pain, um is that saftey? not numbing their mouth and when being uncomfortable, they have three or four doctors standing above them as there scared to death?   • Many of you fail to realize that it is incredibly difficult to treat a child that is thrashing about, screaming, kicking, punching, scratching or biting. Papoose Boards are not to be used on a regular basis and their only use is after Nitrous Sedation or Behavioral Management techniques have failed. This video is incredibly biased because it shows no positive benefits to papoosing a child. What that mother said was completely true, with a tough kid, not papoosing could be more dangerous to kids.

  • Imagine that you are the dentist and you have to place a crown or fill a cavity in a problem child and no other technique works on them. You have tried behavior management, sedation, parental instruction - everything - and they continue to scream and thrash and bite you. Without a papoose board, you are in danger of hurting yourself and the child endangers themselves when there are sharp instruments about.

  • @BusyBeeBen But this isn't what happened. What happened is, the clinic wants to get the job done. They won't bother with taking the time to tell the kid what's happening and calm him. They'd rather RUSH through the procedure and get it done quickly.

    The head dentist said so himself.

  • I'm speaking from experience, by the way, as a dental student. The school I go to sees many underprivileged, incredibly poorly behaved children. We do not papoose them on a regular basis, however. If a child behaved the way Miguel did in the video and we absolutely had to treat, we would be left with no other option. This video could not possibly explain why it was used as it would be a HIPAA violation.

    And notice the end segment where the legitimate dentist said that they never took X-rays of the back teeth that they filled. So they really never checked to see if they had cavities.

  • All of you screaming to kill these dentists or to put them to death are forgetting a very crucial thing - these dentists are working hard to help the child with something that is hurting them from the inside and the child is behaving like a monster. This is not just a normal response to fear or pain - this is a child that is an absolute monster.

    99% of kids I see do not behave this way, and this biased report happened to show the rare instance in which a child was papoosed

  • @ModernWarfareIlvl70 Actually...kids scream and kick mostly out of fear. I've dealt with dozens of children that behave this way, and in nearly all cases, Nitrous Oxide sedation works just fine. It's very rare that behavioral management, nitrous oxide, and alternative calming techniques do not work. He is not screaming because he is strapped down - he's screaming because he's a scared child. Unfortunately, his kicking and punching can hurt him and those working on him, so papoosing him is vital

I already noted that I'm not sure how Small Smiles is dealing with it - I'm simply defending the use of a papoose board *WHEN NECESSARY* (aka NOT routinely).   • @ModernWarfareIlvl70 Hold on. The camera caught an isolated incident that we do not know for sure is appropriate or not. Obviously, this newscast wants to paint a bad picture of the dentist doing it, but we really do not know if that child is crying from being strapped down or because he's a finicky kid. If this news report said "we videotaped the dentist forcefully papoosing this child and then abusing him," I'd be convinced, but all it said was "we videotaped a papoosed child"

  • @BusyBeeBen There's the segments where the head dentist talked about production, and how he said he needed people to get it done quickly, so it's easier to papoose so they can rush through the procedure.

  • @ModernWarfareIlvl70 Also, if you've ever worked on a pediatric patient, you know that they do randomly get very loud, or suddenly freak out. The issue here is that we are given a very minimal sense of what actually happened (because of HIPAA regulations), so we immediately want to sympathize with the screaming child. We don't actually know with certainty that he was not misbehaving. I know that Small Smiles papooses, but I don't know (nor do you) that it's, at least in this case, uncalled for.

  • @ModernWarfareIlvl70 Furthermore, as I said before, this video is showing a one-sided argument. They purposely show a screaming child that is papoosed and then say how bad it is. It's typical emotional reporting.

    My main point is this: Papoosing is necessary when done for the right reasons. It is a gross misconduct if done inappropriately. We do not know if the circumstances called for it or not, but assuming otherwise is unfair to the dentists trying to do their jobs and help sick children

    They showed the Small Smiles videos to other pediatric dentists. The pediatric dentists were sickened

  • Now, in terms of Small Smiles, I cannot say what they are doing is wrong or right - I'm simply putting the idea out there that papoosing is not evil or wrong and that there is a time and a place for it.

    I'm sure many of you will not care and will bash me anyhow and tell me I should be killed or that I'm an awful child torturer.

    From my understanding where real situations where papooses are needed... is a very very small percentage in legitimate clinics

  • @Breamarie91 Because there will be more suffering for the child when the teeth go untreated and become infected. They have all been "numbed" and are only crying wolf. Most kids start crying just becasue you put a finger in their mouth or a little stick with some numbing jelly on it.

  • Anyone is pediatrics would just plain laugh at this video. I'm sorry, many parents might not agree with a papoose but in some cases it is the only way. Some children are not mentally competent enough to understand what and why dental work needs to be done. Some are just that defiant and don't want to listen. Sedations only work on 80% of children, some will just kick and scream straight through it.

  • The only other option is general anesthesia at a hospital where the child is put under as if they were having major surgery. This causes more harm to a child than a papoose will. Any sedation you do on a child as the risk of death. Allowing your child to scream and cry for a little seems to be a better option then losing them all together. If the work is not done then they're mouth will be come infected and with children, a large infection can lead to a hospital visit with IV antibiotics

  • or even death. You can't win them all. Sometimes you have to weight out the bad and the good. Been in pediatrics for 8 years folks, there's not many other ways around it, Sorry!

  • @awoole2 Nope. The other pediatric dentists Baskin interviewed said papooses are hardly ever used.

    Real dentist offices use goofy juices, etc. Plus, the news reported that Small Smiles in Langley Park used papooses for 50% of their patients. Dr. Aldred Williams clearly stated why papooses are used - not cost productive

    And Small Smiles had to pay millions to settle its lawsuits.

  • i took my kids their and had the good sense to walk out they tried to push me into the work saying my son had a lot of work needing done took him to his regular dentist (my son had perfect teeth no cavities) toledo,oh

  • @sabresfan610567 I agree with you and have been working hard for 2 years now, and not one office has closed. However many rules have changed, but they still restrain 95% of all children under the age of 6! Contact your state's dental board, state legislators, and hhs programs and complain.

  • @DebHgn restraints is my problem NEVER should they be used on kids. PERIOD. there are other ways such as being friendly and talking to the child calmly to gain there TRUST not just strap them down and go to work then they grow up fearing the dentist even there doctor. i say outlaw it NOW!!!!

  • Papoose Boards I can see being used for a child with a neurological disorder that causes them to flinch or move for no reason at all.

    However, in these cases it's just torture. This is something I would expect out of a mental institutuon!

  • My 2 boys go to a pediatric dentist. I am ALWAYS allowed to go back with them and they are NEVER restrained. My 10 year old is autistic, and I am even allowed to sit in the chair with him, with his legs draped over my lap. This helps tremendously to keep him calm during the procedure. This practice of strapping down these poor kids is physical AND mental abuse! On another site where parents have discussed Small Smiles, one parent stated that her child had bruises from the papoose board!!!

    though they had to give me an IV to put me to sleep, and even though i was and still am terrified of the dentist, i was never once strapped down or treated without my mum with me

  • plus, if you surf YouTube for first dentist visits, i guarantee you'll find ur fair share of three, four, five, even two year olds who are calm and cooperative without restraint because the professionals communicate with them

    any dentist who says young children need to be restrained automatically are probably just lazy and shouldn't be given money

  • i worked at the small smiles in roselawn of ohio for a year and a half everyone i worked with did not like the rules and policies but we were forced to fallow or we would loose our jobs with the economy and all we were scared i am soo glad i left but please do onot judge the employees judge the corperate and the bad doctors that treat these children like this. i will forever stand by my statements and warn everyone i see NOT TO TAKE THEIR CHILDREN TO SMALL SMILES

I remember being held down in the hospital to be put under general anesthesia. I am 21 years old and I still cry hysterically and freak out before an operation. You are going to scar these children for life. How dare you hold children down like this. SMALL SMILES...YOU ARE IGNORANT ASSHOLES!

  • Lol. You people have no idea what you are talking about. This is why they are Pedodontists. They specialize in out of control kids that need work done. The kids make it seem worse than it really is. They are just scared, nothing acutally hurts. It is especially great that this office takes MA/Blue Plus; most dont and parents pay out of pocket. Most dont have the money and let their kids be in pain from rampant decay.

  • There is no way to stop it. They have an excuse for everything. Sue them, and they will use those excuses. Hopefully they are using anesthetics on those kids, unless those assholes want to answer to me! I saw another video on this, but no one went to prison.

  • I am usually good at the dentist. I cut my head needing stitches, and as soon as I was carried in, I was restrained on a papoose board. It all hit me so quickly, though. I have also been restrained in hospital beds during a few of my stays, and it was traumatic to me at that time. If there are alternate ways to doing treatment, like dental work, work those instead of restraining kids.

  • Talk to your dentist, try to be allowed into the treatment room, ask about the dentists' qualifications and experiences, when your child is sedated and placed on the board, make sure the position is proper for airway/breathing. If you have a couple extra hundred dollars and want to avoid the papoose board, then just go for sedation or pharmacological restraint.
