In Ancient Egypt (real life, not from the show) people believed there was creatures called Kas living inside the hearts of men. So, basically the monsters have ALWAYS been there, but the Millennium Items could remove the creatures whether those monsters were made of goodness or evil.
Aknadin created the Millennium items, but did he create Duel Monsters? If he did not, then who did? Thanks!!
