Okay, the meaning of this joke: The phscicatrist(however you spell it) told him he was crazy.. so Mac said he wanted another opinion (like from another phciatrist (HOWEVER YOU SPELL IT) And the doctor gave him what he asked for, but in another sense. He gave him an opinion of like what he thought of him....And thats not what mac meant... in the joke. so.. thats the joke... in itself.
