When she finished giving birth the baby took too much out of her and she died. Edward injected his vampire venom into her chest to revive her. By morning she transformed into a vampire, so yes, she did die as a human. That's why they did the flashbacks of her life when she met Edward in HS, it was looking back on her life before the change.

I don't think it was just memories that she saw. They say you see your life flash before your eyes when you die, and I believe that was the case in Bella's transformation. I think it was her life flashing before her eyes because these were memories of not only Edward, but her family as well. The majority of the memories were Edward because when she met him. Her life began again. She was dying in a sense, but then being reborn at the same time.
