Uhhhm I know people keep saying that this in MGS2 Substance as an unlockable costume for Raiden and yes you're correct, however if you actually read the description and pause the video when Raiden is first against the wall you can see the modification that shows him with a bloody face. I.e represented when Gray Fox had the laser from Metal Gear Rex slice his arm off and destroy his left eye.
