I don't know this for sure, I'm just guessing...

But I'd have to say that I think what happened was when he died, the game basically had to re-load achievement data...he had the 28 achievements before he did the glitch, you could tell because he had a full Synchronization bar.

The full Synchronization Bar is a default at the beginning of the game. You lose most of it later in the game.

To get rid of the update, and downgrade it do this: go to memory hit memory when you see your hard drive press x x lb rb x x, and it will ask if you want maintence performed hit yes, then turn off your console, and turn it back on.

Well, we need a tutorial really unless it was an accident because you get 28 achievments and you started with 0 achievments which is impossible because to get into Maysaf you have to complete the tutorial mission and you get an achievment for that so makes me doubt its true.
