The new Raiden has basically had his entire Central Nervous System transferred to a new, experimental exoskeleton by the Patriots. I think he was tortured at some point by the Patriots, and used as a test pilot/crash test dummy for the new exoskeleton. Then he escaped, and has gone ronin on the Patriots.

The new exoskeleton has some amazing features. The "high heel" feet resemble tabbi boots worn by ninjas. He can wield a knife between his 2 toes, and the "high heel" allows him to grip things.

They're cyborgs, with machine gun heads, and their legs are made of the recycled bodies of dead soldiers. If you look in that cutscene where snake looks up and sees a gekko with a soldier in its "mouth", you can see that the poor guy's about to be recycled.
