In the movie, the clowns sort of serve as an alarm. Earlier on when ringo accidentally steps on a bagpipe there's a clown that shouts his alarm and spins his head in the same manner.

It's pretty clear from the Hey Bulldog studio video that he is singing the vocal with Paul for the verses on one mike. The rest is really ADT (artificial double tracking) which is a processing of resending the signal but only slightly slower. This is recorded in Feb 1968 and they pretty much stopped singing the vocals twice by this time. humble opinion of course.
