if there r les then 6 people at the end of the game, the all the kills u got in that game wont cout for achievments, just keep going for yr achievment but if theres les then 6 people dont waste time and leave

Check that every act is completed on insane by choosing a chapter and seeing if the 'insane' stars are next to them all. i had the same problem on hardcore. btw. RAMMs a bitch aint he?!
