I got this achievement by shooting the head, both arms, both legs, and the groin.

I got this achievement on mission "Running with the Wolfs" when you're on the ATV's & you gotta snipe out the guys with the M82. But the weird thing is that i sniped out 4 enemies 1 arm 1 leg A torso & lovely last the splattering head. So you could get this achievement earlier.

@sirmidor A person can most certainly die on the spot from that. For example, your heart can go into shock from the fact that you just got your FUCKING arm blown off and die. Plus they may still be alive bleeding out on the ground. Duece is quite a bit away from the enemies

the heart going into shock wouldn't make much difference, since the heart just pumps blood around. the only thing it would do is cause you to bleed out earlier, as i myself called a possibility.

I was having the same problem and after getting 1 x arm, leg, torso and head shot about 20 times over...I finally got it by making a final "GUT" shot. So i think there are 5 body parts that you need to hit. Also, Dusty will say when you have hit each one. "Took his arm off" "Took his leg off" "Center Mass" "Gut Shot" and the head shot icon will confirm the head shot.

@BartOrzel1978 I tried it the EXACT way he is doing it and I didn't get it either. I just gave up and started shooting everyone as fast as I could and somehow got it. Took around half an hour. I think it also includes an ass and/or gut shot. I know one of my shots he said "Ouch, you got him in the gut."
