Well that's open for interpretation; The grave could symbolise the death of their relationship. Or, yes, he could die, and the girl is walking down the path in the park remembering/ getting over him. Like i said it's open for interpretation, so it could be whatever you would like to think happens... but, just so you know, she doesn't die haha
