What you have to remember is that San Francisco is both a city and a county. Normally the county is the governmental entity responsible for public health, free clinics for the indigent, etc. So here you have a county health department which is also the city health department. County health departments got into the HMO business years ago because their employees wanted the health services of the doctors they worked for.
