How were you able to get the true ending when you didn't have the proper souls equipped after beating Grham?

The game only checks for the proper souls equipped on the very last hit of the first phase.

A death counter. As you are going through a room, I would like to see a counter adding up inside of every room with how many deaths were in it. That'd be awesome.
