Carter needs a kidney transplant because he is in renal failure (one kidney stopped functioning after he was stabbed in season 6, and for the other he got an infection while working in Africa).
It wasn't shown on screen. He just explained what happened to him to Dr Banfield during the episode 15.17. You can check the video "The kidney joke" on my channel...

Benton's voice sounds so much more mature and older than the earlier seasons! Kind of sad to realise that many of the cast were on ER 15 years!
