They make hot sauces (technically extracts) that are much hotter than pepper spray. In fact, you can buy pure capsaicin, if you want.

And water does help a lot. The bread that the guy is eating helps the most. Yes, capsaicin is not soluble in water, but anything that rinses or washes it away helps the heat go down.

However, as capsaicin is a non-water soluble oil, you can wash some of it down, but a good amount of it will remain on your tongue unless neutralized by fats (milk) or your saliva. Bread only helps while actually eating it; once it's down, it may as well never have been there.

Capsaicin can not kill you unless you are an infant even then the LD50 is roughly 150mg/kg and that's for a human which weighs 60kg, therefore the amount of totally pure capsaicin (16,000,000 scoville units) you would have to take is around nine grams, quite a hefty amount.
