@METROIDROCKSXP98 That he is. They were basically siamese twins that got seperated against their will by their father and corrupt surgeons when they were children. In the first film, Duane (the "normal" one) would carry Belial (the deformed twin) around in a basket on a mission to kill off the doctors.

In this sequel, they ended up in a house full of deformed folks, and Duane thus had a chance for a normal life... but as tragic events go on, he snaps at the end and does... this. Freaky-ass scene
