If they try to puff up out of the water then their stomachs inflate with air, a condition which can and is usually fatal without human intervention. In the wild, puffing up is not fatal or even harmful to the puffer fish.

@R90S I happen to own puffer fish and I happen to know that they gulp water, and then are just as bouyant as the water so they drift/float around. Gulping air is usually fatal to a puffer fish. For anyone who owns a puffer, please do not ever try to get them to gulp air.

Take 2 small balloons, fill one with air and one with water, then put drop them both in water and see what the water filled balloon does.
