This song is definitely about drug addiction (opiates, heroin, pills) as is the rest of the album. The thirteenth step, obviously refers to the 12 step program. Maynard often writes lyrics from others point of view. Just because he writes a song about heroin, it doesn't have to mean he shoots up. I do think the girl throwing the reptiles in the hole represents the drugs being taken. The more she takes, the bigger the hole in her stomach gets, which means the more she is dying from the drug.
