As far as I know, this could possibly be an unused model that was supposed to be the great fairy. Otherwise, I think this model could also be Navi, a recent interview to the original developers of OoT says that they wanted a really cute girl as Link's companion, but due to the incapacities of the 64 they just used the "ball of light" kind of fairy.
