Lactate threshold depends on the muscle size and blood supply. Fingers may have a LT of <5 watts, arms may be 100 watts, legs 300 watts. That same person using both legs and arms may then be limited by the cardio to 350 watts or so. Someone can have a decent VO2 max from lots of running but may have skimpy legs with a lactate threshold of 150 watts. These people would feel their legs ache on a bicycle but not be breathing that hard. The opposite can be true with big muscles and weak cardio.
@thomashel456 think of it like this, VO2 max is like the fuel and air injectors of a car. Your legs are like the engine size.
With too small muscles, the CV system says, here's lots of food and oxygen, turn it into power!! The muscles are like "Wait up!" The opposite is when the muscles can work hard enough to use energy quicker than their CV system can supply, which is like trying to run a starter motor off AA batteries or a big block V8 from a moped carburettor.
Carmichael is copying what physiologists have been saying for years and years, since the 80s. Lance never used him for training either, it was just for show because their friends. FTP (60 minute power) is the "gold standard".

@TimpBizkit So when an cyclist has a very good VO2max (say 80ml/kg/min), it's better than to build muscles when he 's very skinny?
