Really, it depends on the situation. In everyday speech, I tend not to translate it because a) I'm lazy, b) the amount of space I have for subtitles is limited, c) it's difficult to translate the level of formality in terms of finding a normal English equivalent (e.g. Annie always says "Veigue-san", but "Mr. Veigue" sounds awkward to me), and d) most of the dialogue is spoken, so those who are familiar with honorifics can hear them being used even if I don't translate them directly.
When honorifics are important to understanding the relationship between people though (e.g. Veigue and Claire's parents), I try to indicate that in the subs. And thanks for the heads-up on the titles; this is only my second time through the game, so I wasn't aware of that.
