I've gotten the 1-up fountain to work in actual gameplay. Be small Mario, and stand under the indicated corner as far to the right as possible, then throw the key upward.

Hmmm.. I think it's the way the blocks are placed. I didn't make this game, FuSoYa and Zero-G did. Though I can tell you that the blocks on the left and middle are extra lives. Actually.. I think they ALL are. Try turning them all unto 1-up blocks and lay it out just like that. =P *BTw, Super Bowl just ended.. GIANTS WON! WOOH!*
Lol. Maybe you did it wrong? I think this is how it works: You glitch it so that the key constantly hits the middle block while it's already being hit so since it's technically not a "dead" block it can be hit.. I dunno. Just an assumption. xD Though it's good to hear that you got it without my help.
Doing the things he did? Probably. I assume there were save states and frame rate slow down included, but who knows. With an emulator, almost anything is possible.
In order to edit the overworld and stuff, you gotta use Lunar Magic, a program developed just for those such things. Well, more or less making your own version, but that doesn't count. Shh. xD
It should be pretty self-explanatory, but there are things that you gotta search for. I believe there's a readme with it, so try reading that first.

The 1-up trick DOES work, and I've done it lots of times, and even made a little test level where I did it with coins, stars, turtle shells (quite amusing), and balloons. If you get the information block power-up from the water world secret, it tells you how to do it.
You HAVE to be small Mario. Go completely under the block and release the key straight up. It might take a few tries to get right.

The blue coins are like normal coins except for when you hit a blue swith they turn into blocks you cant walk on and still act like coins
