how the hell do you do the ILS approach for LOWI?? seeing as the course specified is offset by like 15 degrees

Then it's a missed approach. ILS does not land your plane for you, it just helps you get down enough in a safe way so that you can see the runway.

I know, but large planes are also equipped with Autoland which can land for you.

For example, a CATIIIb approach has a descision height of 50ft, or no descision height. And an RVR of 50<200m.

Yes, autoland works great with CATIII approaches (and only some of them). LOWI is then, as you could imagine, not qualified for that. ILS for LOWI is a CAT I approach. Actually the approach is composed of precision and visual approaches.
