Sure it's been altered, atleast in this particular song, but there's some videos on youtube, where u can hear her real voice as she speaks or sings live. She certainly doesn't sound like a man in those either. Many transsexuals might look feminine, but their voice might still be very manly. I don't know how she sounded before the operation so it might be the hormones or that she's just simply very lucky to have a naturally high voice. :D
