Because the first match if you hit Santino and Mark Henry in the head to much, and the game doesn't want them to bleed, so in the first blood match, you just only need to grab the chair and BANG! Done!

Well, McMahon was ringside during the previous match. And I don't know about you, but I took the opportunity to pay Vinny Mac a little visit at that time, so when he attacks you afterward, he's already taken some damage. You might notice that he's already holding his head after the chair is taken away from him, so I believe he carried over the beating from before. Given the new energy system, I think that's probably the most logical reason.
