The blow up prevention device, which is nothing more than a huge rubber washer basically, was compromised when too much drill force was applied via the drill stem. It caused the washer/protector to break apart in bits/pieces. Then these pieces floated to the top of the rig area thru the seal fluid where workers saw them.
