He's in the video, yeah, but he's not really playing. He admits it himself (99.9% positive he does in his book, haven't read it in a while). He says Trent asked him to play guitar in the video and Manson replied, "Well I don't really play guitar" and Trent said it was ok, to just fake it.

I told him, "Well, I don't really play guitar." But I went out there anyway and pretended to play guitar in a video that was never actually released. It was a song called "Gave Up."

"Dried Up, Tied, and Dead to the World", "Minute of Decay", "Rock 'n' Roll Nigger", "Down in the Park", and "In The Shadow of the Valley of Death". There may be another song or two I missed, but those are pretty much it. He's not a very skilled guitarist, and he'll be the first to admit it. He admits it in his book, and this video. So yes, he has played. But he's not a great technical player.
