This sometimes happens when you get a lot of kills fast.  The game tries to give you all the medals, but gets slightly overloaded so it gives you the most recent(ex: killtrocity) then the lower ones (triple kill). It may be fake, but this has happened to me when I got multi Laser kills in big team battle.
