They don't mind killing a lot of people as long as they get to keep having their power. However, I believe that soon they too will face the same fate as those other middle-eastern countries - I think that when Khamenei dies there will be serious demonstrations and riots that will destabilize the country and ultimately lead to the downfall of the government.
