Yes, she is dead. You have be be a little familiar with the concept of Onryo/Yurei to understand. Sadako survives like any other onryo would because she has an intense hatred and will to wreck karmic havoc on all who wronged her, along with some who didn't. She's perpetuating her hate, making everyone suffer with her. With the Onryo class of ghosts in Japanese myths, they creatures of anger/revenge and there is little to no reasoning with them. Spiritual measures are often ineffective as well.

Warning, spoiler! Oly if You have watched Ringu 0 Birthday. Sadako's biological father was the Sea Demon (something like Lovecraft's Yog-Sothoth). Little Sadako had splitted into two girls. One was innocent, shy and authistic, like her mother; and the second "twin" was demonic like her father. Good Sadako was killed by actor's troupe, for her sister's fault. When they took dying Sadako to Ikuma, sisters rejoined and reunited into one person.

Wow, Sadako sure showed a lot more emotion than Samara. That was really good- The Japanese film seems to be a lot more graphic than the American one. (I have only ever seen the American Ring so I can't judge but hey)

Some of this is from the Korean made version, in that one (and the book) Sadako was really a man, he/she looked like a girl but had the genitics of a man, in the Korean one is got raped and the guy found out what she was. Sonata edited that part from The Ring Virus with the end of Ring 0.

Actually, Sadako was a woman but not genetically. Even though she wasn't a transsexual as many would think. The book even states her condition called Testicular Femininization Syndrome or what is commonly now refered to as Androgen Insensitivity Syndrome.
