The uterus is a muscle shaped like an upside-down bag. The baby lives in the bag and there's a hole at the bottom. Dilation is how open the bag is (the opening is called the cervix). In order for the baby to come out, it has to be open 10 centimeters. Contractions make the cervix dilate.

Effaced is a little harder to explain. Basically, it's how thin the bag's opening is. It starts out at around 4 centimeters thick (0%), then slowly thins out to paper thin against the baby's head (100%).
