oh my god! the sounds really get to me. in fact, if it was explicitly shown, it wouldn't have the right effect. the fact that all you see are facial expressions, the gross factor comes with what you hear. you know what's happening, but all you are sensing are grisly slicing and cutting sounds. this is a nasty but ingeniously shot scene.
she actually didn't castrate him, if you watch the whole movie. she told him she drugged him so he wouldn't feel anything, and he believed her, so he thought she was actually castrating him, but she just tricked him so he'd be all freaking out
OK, so I sometimes troll YT for gross out scenes that I am too much of a coward to watch in a whole movie... When the camera pans across him, it looked like he was still wearing his blue underwear, which wouldn't make any sense (unless that was his shirt-tails).
