I would not be surprised that the equipment was being operated much time past it's safety inspection. In my province, crane cables are only to be used for "X many lifts"..(forgot whether it's 100 hours, or lifts) and cabling MUST be replaced..the whole spool. Hinge pins have so many months life.. TOO MANY companies say "We'll replace it"...later...just do this "one more job"...one more day"..."We can't afford it", bullshit... So many men die on the job! To save money by no safety!!!
@RetroCaptain crane cables used for 100 hours or lifts? Not likely as a good operator can acheive 30+ lifts an hour wether it be twin 20's or a single 40 footer.
I'm not aware of the correct time limit placed on safe cable use,..esp in ship loading cranes. My knowledge of cable use limits relates to 1950's-60's truck mounted "wrecking ball" style of crane.
I guess what I'd like viewers to understand, if they don't, is that load-lifting devices require routine inspections,..the hinge pins, bearings,..cables etc have to be replaced before their "useful life" has passed, to increase safety.

That prevents the crane from lifting more than its capable of. All cranes have them. The operator clearly ignored the guys on the ship, you can see them waving to stop. Why were'nt they in direct contact with him on a radio? The boom tip should'nt of broke off either, I heard the operator and a hatch boss died...

Looks to me like the operator thought he could snap it free and it cost him. You can always feel it when you go to make the pick if it's locked up

im a union member crane driver/ dogman and this only happens when the person slinging the load does not know what they're doing and the operator is a cowboy and either goes off chart of falsely believes a load is larger than it is and gets caught undone...

the safest way is the way with the lowest risk involved.
so what if it creates three more job positions for each contractor. in the long run the risks out weigh the cost potential.
