They're all the same person.

Xehanort, Ansem [Xehanort's Heartless], and Xemnas [Xehanort's Nobody] are all Terra. Master Xehanort takes over Terra's body, he becomes Xehanort [his body stays the same, except his skin grows darker and his eyes turn yellow, trademarks of exposure to darkness, his hair also turns silver, I guess cuz Master Xehanort was old.] Anyway, Xehanort is later turned into a Heartless and Nobody, who are Xehanort's heartless and Xemnas.

Technically, both. The only thing about Terra aoutside his body is the Lingering Sentiment.

I hope Xehanort's battle theme is a mix of Forze del Male because it is so epic and dark and the last part of Darkness of the unknown because it is epic as well.

Xehanort's Theme itself should be dark as well. But of course not with so many action as his battle theme.

heres my theory on the matter. Terra hunts down Xehanort (thus the old guy) and beats him, but loses his memory. Ansem finds him and makes him his apprentice (ansem reports) but the only name he could remember was xehanort, so they called him that. over throws ansem, takes ANSEMS name, becomes a nobody, calls himself xemnas, and then dies in KH2. the only problem with my theory, would xehanort/ansem/terra/xemnas has a huge identidy crisis >.>
