i mean what do you base it on/build your improvisation around?

Honestly - I normally have melodies in my head and I am able to put them down. If I don't have something in my head - as I am playing I hear something I like the sound with and go with it. I guess the longer you play the piano, you know what keys sound good with each other. I hope I answered you question.

that the mark of a good improviser is being able to just play what comes into your head instantly

when i improvise i build it around chords most of the time

i have the chords in my head and then build up a melody on top of that

and its true the more you do it the more you know what sounds good together
