I think she still remembers her Ex because that traumatic incident where the Ex's wife attacked her and pulled her hair out (probably spurred her disease) happened after those memories. So anything that happened before that incident is more stable, or something.

A person who has Alzeimers forgets first the present memory then the past.. They suffer anterograde amnesia ( present memory loss) then Retrograde amnesia ( past memory loss) that's why in this film she forgets her husband first and keeps recalling her ex.

so sad but it's true, really happens in real life, though ive seen it with much older people..
