I think the stuff just lets out the true essence of ones being. Poison Ivy didn't go retarded either. The jokers true self is a crazy, insane monster. So he didn't really change much. He just became more of what he already was. Normal people change into more monkey like creatures, because it would make sense. . . . kind of.
