well for me on the last level when your goin up to the bell tower two guys rappel down and one is almost on top the other and then just aim for the top guy and u should get it(this is how i got it0

I did it with the fight with The Immortal, after you damage him, 3 guys come, relatively close to each other, wait for two to line up and Bam!
