She's a Vampire who was locked up shortly after birth. Who locked her up isn't certain, but Remilia kept her like that due to the immense ammount of power that Flandre possesses. The problem is that even though they successfully shielded her from the world, Flandre grew insane after being locked up for so long. Her mental instability and her massive ammount of power causes her to be a dangerous character and very well one of the most powerful characters in the Touhou universe.

though some interpretations just place flandre as a relatively normal, if somewhat sadistic, xenophobe. this idea is from perfect memento, where akyuu mentions that during remilia's garden parties, you can often see flandre freely roaming the halls through the windows.

(personally I like both flandres)

After the events of the Embodiment of Scarlet Devil, it would seem like Remilia loosened Flandre's chains a bit. I still don't think she's anywhere near normal, nor should she be considering how long she has been held locked up. It is also mentioned that pretty much everyone in Gensokyo fears her. I don't think you fear a normal girl, even if she is a sadistic vampire. Most inhabitants of Gensokyo can hold their own against the other inhabitants too.
