I'm not sure but I think this is about her friend/songwriter Townes Van Zandt who died of substance abuse shortly before the "Car Wheels" was recorded. I think "Drunken Angel" was about him as well.

This is one of my very favorite songs. I can listen to it a half dozen times in a row , and often have. What happened to her friend? The lyrics suggest he was on the run, and met with an untimely death? Does anyone here know the whole story?
