The difference is pretty major.

Obama has people who report directly to him, and only him. And they have a significant amount of power to impose upon these other agencies.

Simonson and other advisors to Bush, reported directly to those other agencies and not Bush, and they only had the power assign to them through those agencies.

The problem with Czars is the bypassing of checks and balances.

what you fail to mention is that those 46 people filled only 27 positions. not 46 positions.  in other words, the position of AIDS czar was filled by 5 differnet people but there was only one AIDS czar at a time.

So, the 46 that Bush had over his entire 8 years is far less than Nobama will have over his 4 year term. He has 45 positions for czars, therefore he will most likely have close to 100 czars at the end of his time in the white house.
