"Anvil studio" It's a free music-making program which allows you to make music into MIDIs, unfortunately I can't really explain, but if you look it up on youtube, you'll find some cool songs made with it. :)

That's easy, the song is originally in .xm format, and the file is quite similiar to midi, as in it contains all the original note content. So it's just a matter of opening the file in appropriate program (various trackers, .xm converters etc) and reading the notes.

It's not so easy like that, I try but tracks are impossible to reproduct in 4 or 5 tracks.

Nobody have good result. If anybody have in .mid I take ! : )
