I read from a spoiler guy that works on the set and he said "If you think the first seven minutes were disturbing/shocking, I strongly suggest you skip the remaining 78-odd minutes. Some seriously dark and depressing s**t goes down in that hospital, and its not for the faint of heart. Fun fact: Something big happens in the second hour that doesnt fit into any of the categories in May Sweeps Scorecard."

My guess is its Teddy on a rampage after Owen revealed the truth about her job, his feelings etc in the last episode. The hand holding the gun could be hers and part of me thinks the shoes Bailey can see are there to mislead us. I read somewhere that the shooter is a familiar face as is the main victim (in my opinion-Derek)!
