They take the nipples off, for what I know. If it's a plastic surgery for beauty purposes they have to because they stretch the skin from the sides (i guess up and down too) and then put the nipples back after taking the flacid rest of skin off.

Sorry if I am saying bullshit but that's what i think ^_^
