Another thing about the Nightmare world that it's not just the disturbing loud fucked up moody music. The imagery is fucked up too. For instance those hanging bodies in straight jackets you see in some of the areas. Fucked up!

For anyone wondering what the siren in the game is for and why its used before Alternate Silent Hill, the developers wanted to have a way to introduce the nightmare world with a certain distinction. The siren in the game is an air raid siren, more specifically to refer to the sirens used in civil defense to signal danger from bombings and the like. Air raid sirens in general would instill fear by marking the coming of something dangerous or threatening, so it was used symbolically in this game.
