this is called the undead frank glitch, and its related to being thrown from anything u can ride with 1 life bar off a ledge.

but i still have no idea how you managed to do it without a skateboard or bike, especially at this part of the game.

anyways guys, the game doesnt check death via your life bars. when life bars run out, it triggers a death animation that signals that you died.

If this animation cant play for whatever reason, this happens.
