I hate the use of words 'disproportion' and 'deformed', and that someone 'should' have their nose looking a certain way. According to whose rule book exactly? I don't like the way it's being said. These women don't even have big noses, they just have interesting, unusual ones. If he'd said "What she (the woman in question) wanted was less of a bump/a smaller tip/bridge" then it would have been kinder.

its his job to address the norm of noses because thats what most people want. also, by using harsh words he reinforces the negative self-esteem of potential patients
